Please fill out this brief form to request an Affidavit of Course Completion required to take the licensing exam. A member of our staff will process this request and send a copy of your affidavit to the Center for Guardianship Certification (CGC). You will be CC'd on the email to the CGC.

Please submit only one request; your affidavit will include all successfully completed course hours to date.
